Information forms may be submitted online, dropped off, or mailed to the office. Details not on the form may not be used. Please do not use nicknames and please indicate (if applicable) if your spouse or children are deceased, divorced or remarried.

Photos may be emailed, dropped off, or mailed to the office.
If submitting photos via email, please include your name and place of residence in the message and your full name in the subject line.
If submitting via mail, enclose a self-addressed, stamped envelope, or pick up photos at the office. Fostoria following the appearance in the paper. Photos will remain on file at the office for 3 months.
